Description
The Mental Health Therapist/Social Worker 2 – Concurrent Disorders provides comprehensive, client-centred mental health and addictions services to individuals across a range of populations, with a primary focus on adults experiencing mental health and substance use concerns. This position delivers evidence-based psychotherapy, counselling, comprehensive psychosocial and addiction assessments, crisis intervention, and treatment planning to support clients in achieving their recovery goals.
This role also facilitates a variety of therapeutic and psychoeducational groups within community-based programs, fostering skill development, resilience, and recovery in both individual and group settings. In addition, the incumbent provides ongoing case management and care coordination, collaborating with internal and external agencies, and support networks to ensure clients receive integrated and client-centred care.
